Due to the current state of the COVID-19 pandemic, we ask that all clients please fill out the Wellness Screening and Treatment Form before booking an appointment.
It is required 24 hours advance notice to reschedule or cancel your appointment. If you fail to notify or do not show up a Cancellation Fee of $25 will be applied. Cancellation Fee must be paid before scheduling next appointment.
